Metallica staat bekend om het doen van experimenten en maakte van het 9e album iets ongewoons. S&M werd opgenomen tijdens twee concerten in Berkeley, samen met het San Francisco Symphony Orchestra. De muzikanten van Metallica beperkten zichzelf niet tot de langzamere nummers, maar speelden ook nummers zoals "Hero Of The Day" en "Master Of Puppets". De titel S&M is een woordspeling van de afkorting die staat voor sadomasochisme. In de context van het album, staan de letters voor symfonie en Metallica. Dit wordt uitgebeeld op de cover, waar de S een gespiegelde muzieksleutel voorstelt en de M natuurlijk het bandlogo.
